日本は世界屈指の温泉大国で、温泉施設の数は2万を超えるそうです。日本人は温泉には裸で入るものだと思っていますが、外国人にとってはとても恥ずかしいことのようです。
今回のフレーズは、温泉に初めて入る外国人の気持ちも表している面白いフレーズです。

足が冷たくなる事ではなく、急に躊躇する事です。気が引ける時は、やるつもりだった事をやめたい気持ちになります。
She's getting cold feet about going to the ocean in a swimsuit. は「彼女は水着で海に行くのは気が引ける。」と言う意味で、彼女は行くか行かないかを迷っているというニュアンスになります。
